Annual Hiring of an Insurance Company to provide Insurance Services for the TB warehouses where Tb drugs are domiciled in the zonal stores.
PRE-QQUALIFICATION
Your company must be a registered Insurance Company with the requisite experience and track records for provision of Insurance. The documents below must be made available in a sealed Technical Bid (the scoring for each is stated and will be used as such in the selection); Please note that the Financial bid will not be opened if the score for the pre-qualification is below 70% score)

(2.0) ELIGIBILITY REQUIREMENTS
Bidders are to provide the following as minimum qualification requirement for the Project(s) of their choice.
(2.1) STATUTORY REQUIREMENT
(a) Evidence of Incorporation of the Company with Corporate Affairs Commission (and updated Annual Returns: Article of Association; form C02; and form C07.)
(b) The company must show evidence of fulfilling its Tax obligations as follows:
(i) Current Tax Clearance Certificate (TCC) for the last three (3) years ending 2015; and TIN Number (Tax Identification Number).
(ii) Audited Account for the last three (3) years duly endorsed by a Registered Accounting firm.
(iii) Evidence of financial capability (Statement of Account or Turnover) and reference letter from a reputable bank.
(c) Evidence of possession of PENCOM of Compliance for 2016.
(d) Evidence of Registration on the Bureau of Public Procurement National Database of Contractors, Consultants and Service Providers.
(e) Evidence of fulfillment of Statutory Contributions to the Industrial Training Fund (Certificate of Compliance) and Nigeria Social Insurance Trust Fund.
(f) A Sworn Affidavit in line with section 6 of the Procurement Act 2007.
(g) A letter authorising the Authority or her representative(s) to conduct Due Diligence on all documents submitted by the company from any source with the view of confirming their genuineness or otherwise
(2.2) TECHNICAL REQUIREMENTS
(A) Evidence of work experience as the main contractor executing or having executed a minimum of four (4) projects of similar complexity, compared to the proposed work, one of which must have been within the last 3 years. Scanned copies of award letters and/or interim Certificate of Valuation for on-going project(s), Completion Certificate and/or Final Maintenance Certificates where applicable are to be enclosed.
(B) Evidence of staff holding of at least four (4) relevant professionals, stating their roles, designation and experience.
(C) Evidence of possession of relevant machinery and equipment.
(D) History of company’s policy on Community Social Responsibility with evidence.
(E) Methodology of implementation for the project.
(2.3) Additional Requirements for Consultancy Services (LOT 15)
(a) Evidence of having executed works of similar complexity and size within the last five Years.
(b) Prospective Consultants should have a minimum turnover of at least Three Hundred Million Naira (N300,000,000.00) in the last Three (3) years.
(c) Prospective Consultants should collect Expression of Interest Form from the Procurement Unit at no cost.
In addition, scan copies of certificates of Technical/professional staff of the company to be submitted are to be initialled, endorsed by the owner and professional seals embossed on the certificates.

(ii) The Technical Bid should be accompanied with a copy of the official receipt of purchase of bid documents.
(iii) No change of Lot/Return of tender processing charge will be entertained after the issuance of receipt.
(iv) All completed bids should reach the Office of the Managing Director on or before 10.00am, Monday 25th July, 2016.

(xii) Verifiable list of previous similar works successfully carried out within the last three years with names of client, evidence of award and job completion.
(xiii) Company profile including among others, organizational structure and key professional staff with their names, and telephone numbers, at least one of which must be a COREN registered Engineer with 2016 Practicing License (For Lot 1 and 2).
(xiv) Audited annual financial report of the company for the last three years (2013 -2015)
(xv) Evidence of financial strength, credit worthiness to the tune of not less than N10m (N3m for Lot 3)/bank reference in addition to evidence of a turnover of at least N50m (N10m for Lot 3) in the last three years (2013-2015).
(xvi) The offer must be properly signed.
(xvii) All schedules provided in bid documents must be completed.
(xviii) All bids must be submitted in English language.
(xix) Documentary evidence that the bidder accepts the General and Special Conditions of Contract as indicated in all the forms provided in the bid document.

(v) The Agency will not enter into unnecessary correspondence with any Consultants except when deemed fit and in the interest of the bid.
(vi) All costs incurred by any Company in respect of this advertisement are at her own expense and the Agency shall not entertain any claim whatsoever in this regard.
(vii) The Agency reserves the right to verify any claim made by any of the Bidders.
(viii) The Agency reserves the right to suspend or cancel the bidding process at any stage if and when the need arises.
(ix) Late submissions will be rejected.
Non adherence to any of the instructions above attracts automatic disqualification.

(2a.) MANDATORY/ELIGIBILTY CRITERIA:
Prospective bidders are required to submit the following documents:
(i) Certificate of Incorporation/ Registration with Corporate Affairs Commission (CAC);
(ii) Evidence of Current Certification /Exemption from PENCOM in accordance with the Pension Reform Act, 2004 including evidence of remittances of Employees’ Pension Contributions/ Deductions;
(iii) Evidence of Tax Clearance Certificates for the last three years (2013, 2014 & 2015) corresponding with declared turnover;
(iv) Evidence of VAT Registration with TIN No and past remittances for the last 3 years;
(v) Evidence of Current payment of training contributions to Industrial Training Fund (ITF) in compliance with the provisions of ITF Amendment Act 2011;
(vi) Evidence of registration with Financial Reporting Council;
(vii) Evidence of Current Registration with Bureau of Public Procurement (BPP)
(2b.) OTHER REQUIREMENTS
(i) Company Audited Accounts for the last three years (2013,2014 & 2015) showing annual turnover;
(ii) Verifiable list of similar jobs successfully executed in the last three years including letters of award of contracts, project costs, job completion certificates (please provide contact addresses of the Clients);
(iii) Evidence of Financial Capability to execute the Projects (Bank Guarantees from reputable banks will form part of the evaluation criteria);
(iv) Company Profile and Technical qualifications of key personnel with evidence of experiences on similar jobs;
(v) For all building Construction projects, bidders must show evidence of Staff registered with CORBON;
(vi) Any bid above 300 million for building construction must be accompanied with Bid Security of 2% of Bid Price;
(vii) A sworn affidavit confirming that all information presented and attached are true and correct of the company and that no officer of NAHCON is a former or present Director, Shareholder or has any pecuniary interest in the bidder.
(3) COLLECTION OF TENDER DOCUMENTS
Tender documents are obtainable from the office of the Head of Procurement, NAHCON Room 207, 1st Floor on the presentation of a copy of the original receipts of non-refundable tender fee of N25,000.00 (Twenty Five Thousand Naira) Bank draft issued in favor of National Hajj Commission of Nigeria (NAHCON), Sokoto House, No 1 First Avenue Off Ahmadu Bello Way, Central Business District, PMB 375, Garki Abuja for each project lot.
(4) SUBMISSION REQUIREMENTS:
(i) Completed Technical and Financial bids (three set of hard copy) shall be submitted in two different sealed tamper proof envelopes and labeled “Technical bid” and “Financial bid” and both placed in a third envelope. All the 3nos. envelopes should bear the “Project Name” and Project Lot No. at the Top Right Hand Corner of the Envelope and addressed to:

(ii) The documents should be deposited in the Tender Box at the Tender Secretariat, Room 207 1st Floor, Sokoto House, No 1 First Avenue Off Ahmadu Bello Way, Central Business District, PMB 375, Garki Abuja, Company names should also be written at the reverse side of the envelope. Closing time for all submissions is on or before Monday, 25th July, 2016 at 12.00noon prompt. Late submissions shall not be entertained.
(5) OPENING:
Opening of the received documents will commence immediately after the dead line at the NAHCON’s Conference Room on the 2nd Floor. All Bidders and/or their Representatives, Relevant Professional Bodies and NGOs are invited to witness the public opening exercise accordingly.
(6) PLEASE NOTE THAT:
(i) All CAC, VAT, PENCOM and TAX Certificates may be referred to the Security Agencies, Corporate Affairs Commission, Federal Inland Revenue Services (FIRS), National Pension Commission and other relevant bodies for verification.
(ii) Contractors are not permitted to Bid for more than two Project lots and only successful bidders will be invited for the next stage of the bidding process.
(iii) NAHCON reserves the right to reject any or all the submissions that do not meet up with the requirements as specified.
(iv) The Evaluation of the received documents shall be strictly based on the requirements as specified above.
