(3.0) ELIGIBILITY CRITERIA
Firms interested in bidding for any of the jobs must possess the following eligibility requirements:
(i) Evidence of Registration with the Corporate Affairs Commission (CAC).
(ii) Detailed Company Profile showing evidence of relevant equipment and evidence of key personnel.
(iii) Tax Clearance Certificate for the last three (3) years (2013, 2014 and 2015) with Tax Identification Number (TIN).
(iv) Valid PENCOM Compliance Certificate.
(v) Valid Industrial Training Fund (ITF) Compliance Certificate.
(vi) Interim Registration Report (IRR) as evidence of registration on National Database of Contractors, Consultants and Services Providers in line with Section 16(7) of the Public Procurement Act (PPA), 2007.
(vii) Audited company account for the last three (3) years (2013, 2014 and 2015).
(viii) Evidence of similar jobs completed/on-going by the company.
(ix) Evidence of financial strength, credit-worthiness and solvency from company’s Banker(s).
(x) A sworn affidavit in line with the provisions of Part IV, Section 16, Subsection 6(f) of the Public Procurement Act, 2007 stating that:
(i) All documents/information presented are genuine and verifiable
(ii) The Director(s) of the firm has/have never been convicted by any court of law within or outside Nigeria
(iii) None of the company’s Directors or the company itself is bankrupt
(iv) None of the officers of University of Benin Teaching Hospital (UBTH) or Bureau of Public Procurement (BPP) is a former or present staff of the company
(v) The firm is not in receivership
These documents are to be arranged in the order in which they are listed.
(4.0) COLLECTION OF BIDDING DOCUMENTS
Interested companies are to collect the Tender Documents from the Procurement Division (Room 9 for Supplies and Room 14 for Works/Construction)’, Administrative Block, (Laundry Extension) from the date of publication of this advert between the Hours of 9:00am and 4:00pm daily with evidence of payment of a non-refundable fee of N30,000.00 (Thirty thousand naira) only per lot. This applies to only those firms that did not participate in the last bid, the sum is to be paid through Remita Platform to UBTH at Skye Bank Plc., UBTH premises, Benin City and a copy of the receipt presented before the collection of Tender documents.
(5.0) SUBMISSION OF BIDDING DOCUMENTS
Tender documents are to be enclosed in two separate envelopes, one to contain the Technical Bid and boldly marked “Technical Bid” on the top right corner and the other to contain the Financial bid and boldly marked “Financial Bid” on the top right corner. The two envelopes are to be placed in one large envelope which should be addressed to:

All Tender documents are to be submitted not later than 12:00noon, on Tuesday 16th August, 2016. Bids submitted after the deadline for submission shall be rejected.
All submissions are to be made in English Language.
(6.0) OPENING OF BIDDING DOCUMENTS
The bids shall be opened immediately following the deadline for submission on Tuesday 16thAugust, 2016, by 12:00noon at the VIP Lounge, Catering Department, UBTH, Ugbowo, Benin City. Bidders or their representatives as well as interested members of the public are invited to witness the opening of the bids.

The University of Benin Teaching Hospital is not obliged to award any of the contracts to the lowest cost bidder.

(iv) Annual Audited Account for the last three (3) years (2013, 2014 and 2015)
(v) Name, e-mail address and telephone numbers of contact person for the project;
(vi) Affidavit of disclosure that none of the persons connected with the bid process in the procuring entity has any pecuniary interest and that the company is not in receivership of any form of insolvency/bankruptcy nor in debarment and that the company nor any of the Director(s) of the company been convicted of financial crime;
(vii) Any other information that will further enhance the qualification of the company;
(4.0) COLLECTION OF TENDER DOCUMENTS:

Completed Technical and Financial Documents (3 Sets of Hard Copy) for category B and C shall be submitted in Two different sealed tamper proof envelopes and labeled “Technical Bid” and “Financial Bid” and both placed in a third envelope. EVALUATION CRITERIA
Companies are expected to fully comply with the above requirements as failure to do so may weaken the company’s submission. All submitted responses would be evaluated on the basis of documents submitted and selection would be based on industry best practices. These may include (but are not limited to) company’s competence, experience, track record of performance etc.
