(2) Eligibility/Mandatory Requirements:
(i) Certificate of Incorporation from Corporate Affairs Commission.
(ii) Certificate of Compliance from PENCOM in accordance with the Pension Reform Act 2004.
(iii) Tax Clearance Certificate for the immediate past three years (2014, 2015 & 2016).
(iv) Certificate of Compliance with Industrial Training Fund (ITF) Act, 2011.
(v) Interim Registration Report (IRR) from Bureau of Public Procurement (BPP).
OTHER REQUIREMENTS:
Manufacturers authorization from the approved Manufacturers listed in the Policy for Procurement of medical equipment.
(vii) Company Audited Accounts for the past three years (2013, 2014 & 2015).
(viii) Company profile containing list of key personnel’s and ownership or availability of equipment’s where applicable.
(ix) Completion certificate for three (3) similar jobs successfully executed within the last three years.
(x) Reference letter from reputable banks with bank statement of accounts (Jan-Dec 2016) /evidence of availability of credit facilities (not less than 50 percent of quoted price) from a reputable bank.
(3) BID COLLECTION: Bids documents can be obtained from the Office of the Secretary Tenders Board. Room 04 Admin Block with a copy and original receipt of non-refundable Tender fee of N10,000.00 (Ten Thousand naira) paid into National Obstetric Fistula Centre Abakaliki (Remita Account) for each lot.
(4) SUBMISSION OF BID DOCUMENTS: Completed Technical and Financial Bids (three sets of hard copy and 1 soft copy) shall be submitted in two different sealed tamper proof envelopes and labelled “Technical Proposal” and “Financial Proposal” respectively, and both placed in a third envelope. All the 3 nos. envelops should bear the “Project Name” and Lot No. at the Top Right Hand Comer of the Envelop, Company Name at the reverse side and addressed to:
Medical Director, National Obstetric Fistula Centre Abakaliki, 86 Abakaliki-Enugu Expressway, Abakaliki, Ebonyi State.

The documents should be deposited in the Tender Box at the Admin Complex of National Obstetric Fistula Centre Abakaliki.
(5) CLOSING DATE AND TIME FOR SUBMISSION OF BID DOCUMENTS
Closing date/time for all submissions shall be 12 noon prompt on Monday 2nd May, 2017.
(6) BID OPENING: Technical Bid Documents shall be opened in public immediately following the deadline for the submission of bids at the Admin Complex of National Obstetric Fistula Centre Abakaliki.

1.0 PREAMBLE

• Evidence of compliance certificate with the provision of Section 6(1)13) of the amended Industrial Training Fund (ITF) Act No. 19 of 2o11
• Evidence of compliance with all NSITF/Payment of Training contributions to the Industrial Training Fund (for year 2016)
• Companies audited account for 3 years i.e. 2013, 2014 and 2015. Please note that the Turnover in the Audited Account must match Turnover declared in Tax Clearance Certificates.
• Evidence of Registration on the Bureau for Public Procurement (BPP) National Database of Contractors, Consultants and Service Providers
• Profile of the Company including list of similar projects executed, Letters of Award/completion certificates and curriculum vitae of professionals in the company’s employment.
• Evidence of financial capability supported with bank statements for the last six months from a reputable bank.
• Every submission must be accompanied with an affidavit that all information and documents submitted are true and correct and that no officer of DBI is a former or present director, shareholder or has any pecuniary interest in the bidder. Please note that copies of Tax Clearance and PENCOM Certificates will be forwarded to relevant Government Authorities for validation by the DBI in the course of the bidding process.
• All documents listed above must be arranged in the order of listing in both the original and duplicate copies.
4.0 COLLECTION OF TENDER DOCUMENTS / INSTRUCTIONS
The collection of Tender Bid attracts a tender fee of N25, 000.00 (Twenty-Five Thousand Naira Only) through the bank payable to Digital Bridge Institute, Zenith Bank. Account Number: 1010797843. Evidence of payment of the tender fee must be attached when returning the Tender Bid.
The prospective bidders shall enclose the technical and financial bids m two (2) separate envelops and enclose the two envelopes in one sealed envelope clearly marked Financial and Technical Bid on the top right-hand corner. 5.0 SUBMISSION OF TENDER DOCUMENT

PRE-QUALIFICATION REQUIREMENTS
THE FOLLOWING DOCUMENTS ARE TO ACCOMPANY THE BIDS;
(1) Evidence of Registration/Incorporate with Corporate Affairs Commission (CAC);
(2) Evidence of Tax Clearance for the last three (3) years (2014, 2015 & 2016);
(3) Profile of firm including ownership. Management Team, and list of Equipment/Materials etc.;
(4) A list of similar contract carried out by the firm attested by contractors LPO’S covering at least three years;
(5) Evidence of Compliance with Industrial Training Fund (ITF) contribution;
(6) Evidence of Compliance with Pension Fund — PENCOM Act 2004;
(7) Evidence of registration on the National Database of Contractors, Consultants and Service Providers by (BPP) or a copy of Interim Registration Report (IRR) as evidence of compliance with the requirement;
(8) Evidence of Compliance with the National Social Insurance Trust Fund (NSITF);
(9) Company profile showing organization structure, names and resume of key professional staff and their qualifications;
(10) Company Audited Accounts in the last three years (2014.2015&2016);

(12) Sworn Affidavits.
Only contractors with proven capability, relevant and good records of accomplishment would be considered based on satisfactory assessment of Prequalification documents and reports of projects verification exercise by the Institution Technical Committee.
COLLECTION OF PREQUALIFICATION/TENDER DOCUMENTS
Prequalification/Tenders Documents will be available for collection between 8.00am and 4.00pm from Monday to Friday in the office of the Director of Physical Planning upon payment of non-refundable fee of N30,000.00 at the Bursary Department for Tender documents.
SUBMISSION OF PREQUALIFICATION AND TENDER DOCUMENTS
The Submission of documents must not be later than 12noon of Monday, 24th April, 2017. Both the Prequalification and Tender documents will be opened at 1.00pm on the closing date in the Polytechnic Multi-purpose Hall.
• Completed Tender documents must be waxed sealed in two different envelopes labeled Financial or Technical with the Company names boldly written on the back of the envelopes. Both envelopes will be placed in a third one sealed, labeled and addressed to the Ag. Director- of Physical Planning.
• Late submission will be rejected.

INVITATION FOR EXPRESSION OF INTEREST (EOI) FOR INVESTORS TO LEASE FEDERAL COASTAL FISHERY TERMINALS IN EBUGHU, AKWA-IBOM STATE; BOROKIRI, RIVERS STATE; AND IGBOKODA, ONDO STATE

(1.0) BACKGROUND
(1.1) In pursuance of the present Administration’s Agricultural Promotion Policy (The Green Alternative) and the need to ensure self-sufficiency in domestic fish production, to promote food and nutrition security in the Country, the Federal Ministry of Agriculture and Rural Development intends to promote standard post-harvest fish handling and processing through the establishment of coastal fishing terminals in targeted coastal States in the Country.
(1.2) In realization of the above objectives, the Ministry intends to lease its existing Fishery Terminals at the following locations:
(i) Ebughu Fishery Terminal, Akwa Ibom State
(ii) Borokiri Fishery Terminal, Rivers State
(iii) Igbokoda Fishery Terminal, Ondo State
(1.3) The purpose of the Terminals were to provide shore based services to the Nigerian Industrial Fisheries. The Terminals have some basic infrastructural facilities such as:
(i) Jetty/Water front for vessel mooring and fish discharge
(ii) Shipway/Drydock for vessel repair
(iii) Engineering workshop, housing machineries
(iv) Fuel Dump with Storage Tanks for Storing Automotive Gas Oil (Diesel)
(v) Canoe ramp to service artisanal fishing boats
(vi) Powerhouse
(vii) Cold-room for Fish storage
(viii) Borehole and water storage Tanks as well as a water treatment plant-for fresh water supply
(ix) Ice-making plant
(x) Ware house for storage of utilities
(xi) Administrative blocks – for offices.
(1.4) Consequently, Expression of Interest (EOI) is hereby invited from suitable Industrial Fisheries Operators, Corporate Bodies, Investors, (Investors may be Natural Persons or Cooperatives, Fisheries’ Associations, entities) that are interested in the lease of these facilities to participate in the scheme.
(2.0) SPECIFIC REQUIREMENT FOR EXPRESSION OF INTEREST (EOI)
(2.1) Investors must provide information indicating that they are qualified to participate in the scheme. Interested investors are to submit written application supported with the following:-
(i) Certified true copy of Certificate of Incorporation (for Corporate entities only);
(ii) Detailed Company profile (for Corporate entities only);
(iii) Evidence of Identification e.g. National Passport or Business name, National Identity Card or Voters Card (applicable for Enterprises/natural persons, only);
(iv) Maximum of two (2)-page brief covering relevant experience in fisheries sector;
(v) Expression of Interest (EOI) should be specific and limited to only one (1) location.
(vi) Evidence of possession of adequate technical and managerial capacity/experience in managing .similar facilities, or partnership with persons with such capacity.
(vii) The ability to meet up with at least 25% of the total financial cost.
(viii) Audited account for the last three (3) years for corporate bodies.
(ix) Any other document(s) to support the application.
(3.0) SUBMISSION OF EXPRESSION OF INTEREST (EOI)
(3.1) The Expression of Interest (EOI) applications are to be submitted in three (3) copies including one (1) original and two (2) photocopy, properly sealed and clearly marked at the top right hand corner “Expression Of Interest (EOI) for Investors to Lease Federal Coastal, Fishery Terminals In Ebughu, Akwa-Ibom State; Borokiri, Rivers State; and Igbokoda, Ondo State”
(3.2) The sealed envelopes should be addressed and delivered to the address below:

(iii) The Ministry will set-up Monitoring Team to confirm the capacity of the core investor to manage these facilities.
(iv) The Ministry will not be responsible for any cost or expenses incurred by any interested investor in connection with any response to this invitation and or the preparation or submission in response to an enquiry.
(v) The Ministry is not bound to accept any bid and reserves the right to annul the bidding process at any time without incurring liabilities in accordance with Section 28 of the Public Procurement Act, 2007.
(vi) Contravention of any of the conditions state above could lead to disqualification
