(x) List and verifiable documentary evidence of at least three (3) similar job(s) successfully executed in the last five (5) years including letters of awards and job completion certificates;
(xi) List and verifiable evidence of construction equipment with proof of ownership or lease agreement;
(xii) All documents for submission must be transmitted with a covering/forwarding letter under the Company’s letter head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact telephone numbers, valid e-mail address and official Company’s address.
(xiii) The letter head paper must bear the name, nationalities and the telephone numbers of the directors of the company at the bottom of the page;
(xiv) Current sworn affidavit disclosing:
(a) That the company is not in receivership, insolvent or bankrupt,
(b) That the company does not have any director who has ever been convicted in any court in Nigeria or any other country for criminal offence in relation to fraud of financial impropriety,
(c) Whether or not any officer of the federal ministry of education is a former or present director, shareholder or has a pecuniary interest in the bidder, and
(d) That all documents submitted are true and correct in all particulars.

Interested consultancy firm must have the following minimum qualifications:

(C) Pre-Qualification Requirements
Interested and competent Contractors wishing to carry out the job are required to submit the following documents which will be subjected to verification by CRIN.
(I) Evidence of registration with Corporate Affairs Commission (CAQ;
(II) Evidence of Current Tax Clearance Certificate for the last three (3) years; 2014, 2015,2016;
(III) Evidence of VAT registration/remittances; Bank reference and evidence of financial capability;
(IV) Company Audited Account for the last 3 years (2014,2015,2016);
(VI) Company profile with curriculum vitae of key officers, including photocopies of relevant professional/technical qualifications;
(VII) List of verifiable construction equipment indicating ownership or lease agreement;
(VIII) Verifiable evidence of successful completion of similar works with copies of Letters of award and certificates of successful completion;
(IX) Evidence of Compliance with PENCOM or Pension Commission in accordance with Pension Reform Act 2004 (as amended);
(X) A sworn affidavit disclosing whether or not any officer of Federal Ministry of Agriculture and Rural Development, Bureau of Public Procurement or Cocoa Research Institute of Nigeria, is a former or present Director, Shareholder or has any pecuniary interest in the bidder and confirm that all information presented in its bid are true and correct in particular.
(XI) Evidence of compliance with the amended Industrial Training Fund Act 2011 by inclusion of compliance letter from ITF and stated to be valid upto31 Dec., 2017
(XII) Verifiable Certificate of Registration with the Bureau of Procurement (BPP) National Database of Contractors, Consultants and Service Providers.
(XIII) Evidence of compliance with National Insurance Trust Fund (NSITF)
N.B: The above listed requirements shall form part of the bids evaluation criteria and non-compliance with any of the stated conditions shall result in automatic disqualification of the bidder.
